If you have concerns about a student or need to report an incident, please use the forms below to ensure the information is directed to the necessary individuals.

If the incident threatens the health and/or safety of an individual or the College community or emergency medical care is necessary, call 911 IMMEDIATELY.

Complaint Form

Student, employee and community concerns, grievances or complaints.

Report IT!

Accidents, injuries, illness, property damage, loss or theft.

Academic Integrity

Cheating, misrepresenting work as their own or plagiarizing.

Student CARE

Academic, social, behavioral, physical, mental health, well-being, basic needs or financial concerns.

Hazing

Activity that humiliates, degrades, abuses or endangers a person to join or continue in a group, regardless of willingness.

Student misconduct, concerning or suspicious behavior.
